Self-Surveys are sent as a hyperlink by email and/or text. This survey is meant to be completed on a mobile device. When you tap the link, you'll see a screen similar to the one below. If your screen says Smart Consult, see the instructions below.

When you reach the Room Recording screen, tap the room you'd like to record first.

You'll be prompted to give access to your camera and microphone to complete the survey. Tap Allow or Allow while visiting the site, depending on your device.

You're now ready to go. If you navigate away or tap Never allow, a notification appears at the bottom of the screen. Tapping a room will bring the permission prompt back so you can grant access.
